Check out the UC-IPM website and take a look at the insect section. That looks like the work of beetle or maybe moth larvae, but otherwise I can't tell.

There is so many different species of beetle that's grub would eat through trees that without a specimen it's almost impossible to figure out which one did it. But yes I think that killed the tree, or at least let in some form of fungus into the tree that killed it.
