Egg -> Larvae - what actually happens

Egg to larvae seems to always be described as hatching. Is this the case? ie does the first stage larvae grow in the egg and the egg then hatches discarding its shell or is it more of a morphing thing? 

I have seen the transitions from larvae to pupae (with cocoons) and eclosing but was always curious about this!

Yep. The egg is the same as any other insect egg, larvae develops inside and then hatches out of the egg shell. You can sometimes see discarded eggshells if you're observant.
